Forms required prior to child's attendance:
Preschool/STEAM Laboratory Enrollment Form
Child Health Report (This form must be signed by your child's doctor.)
Authorization to administer medication (form only required if we are to administer medication to your child, while they are in our care).
Preschool students will also need to have the Preschool Registration form completed.
