Christ the Life Preschool provides monthly calendars for scheduling children. The calendars are available next to the sign-in computer or you may print one from the website. Please turn in the calendar 6 school days in advance to avoid drop-in rates. Changes in the monthly calendar can be made by using the Revised Schedule form.

Forms needed to start Preschool or Childcare |

The state of Wisconsin requires the following forms to be completed and returned to the school office prior to the attendance of Preschool. The Child Health Report requires the signature of a physician. Print out the PDF files, fill out completely and return to the school office. Thank you. |
